For many women, a mammogram is the first step in proactive breast health. However, a mammogram provides a two-dimensional view that may not always offer a complete picture, particularly for those with dense breast tissue. In our practice, we often recommend a screening breast ultrasound as a vital secondary tool to ensure a comprehensive evaluation

Understanding Breast Density
Breast density refers to the ratio of glandular and connective tissue to fatty tissue. On a mammogram, fatty tissue appears dark, while dense tissue appears white. Because potential abnormalities or tumors also appear white, they can sometimes be masked by the surrounding dense tissue. This “masking effect” is a primary reason why supplemental imaging is considered for many women.
When an Ultrasound is Recommended
A breast ultrasound uses high-frequency sound waves to produce real-time images of the internal structures of the breast. We typically find an ultrasound necessary in the following circumstances:
- A prior mammogram has identified the presence of dense breast tissue.
- There is a need to distinguish between a fluid-filled cyst and a solid mass.
- A patient has a personal or significant family history of breast cancer that necessitates higher surveillance.
- There is a specific area of concern or a palpable lump that requires further characterization beyond what a mammogram can show.
Why Ultrasound Complements Rather Than Replaces
It is a common misconception that an ultrasound can replace a mammogram. In reality, the two technologies work together. Mammograms are the only reliable way to detect microcalcifications, tiny calcium deposits that can be an early indicator of cancer. An ultrasound cannot reliably see these. Conversely, the ultrasound can “see through” dense tissue in ways a mammogram cannot. By utilizing both, our clinical team can achieve a much more detailed and accurate assessment.
